Annul the knighthood of Sir David Nicholson.
He was the chief executive of the West Midlands Strategic Health Authority, which oversaw the Mid Staffs NHS Foundation Trust. Mismanagement at the trust is thought to be responsible for up to 1,200 deaths.
By comparison, Fred Goodwin's knighthood was annulled even though nobody died.
