poor NHS,Social service
My father has been discharged from hospital on 19/10/11.
Father has following clinical illness and is disabled.
1: Stoke (Carotid arteries blocked 100% one site, 90% another side)
2: Heart Failure on pace maker for over 2 years
3: In ability to stand up on his own and walk to commode's or toilet.
4: Prostate
5: Diabetes
6: Breathing problem
7: In ability to manage his daily needs.
8: Living on own.(we cook food for him for carer to help him to eat and provide his medication)
Since his discharge following careless or negligence have taken place.
GP has not reviewed his medication yet.
Neurosurgeon appointment for his Carotid artery has not yet been made.
We believe if care is not sorted out urgently, our father will die due to professional negligence of Social services, Doctor and Nurses.
