Introduce a (5%) Voluntary Tax where people can allocate as they choose

There is a lot of shouting about the lack of funds here and there. NHS, education, police and soforth. But no-one especially wants a tax raise, especially when their funds are tight. What this idea is a voluntary tax, but one where a person can specifically choose where that tax goes.

Additional Information

The way it would work it would be something that people can opt-in, or opt-out. Opt-in of a voluntary tax, and they would then pick a choice where they want that payment of tax to be allocated. For example.
>NHS >Education >Roads >Railways >Military etc.
