A PUBLIC INQUIRY to re-establish public confidence in the roles of the various professions concerned with the safety and protection of vulnerable children and young people with particular emphasis on:

the roles of Inspections (internal and OFSTED) into Social Services Departments together with the experience of people carrying them out;

any adverse impacts of Performance
Indicators (PI's) on Safeguarding;

the qualifications and qualities required of Directors of Children's Services;

the level and quality of formal Supervision offered within all agencies;

the minimum staffing levels and 'skill mix' of staff employed within all relevant frontline agencies;

the role of the Media and whether greater controls over known inaccurate reporting should be exerted;

any other actions required to better understand why such tragedies continue to occur and whether there is evidence of suppression of factual evidence.
