Maternity Exemption Certificate extension
I would like the Maternity Exemption Certificate
To be extended for dental treatment for women who are currently pregnant or on maternity leave
While pregnant and on maternity you would normally get an Maternity Exemption Certificate for dental treatment. However this has been unable to happen.
