Additional Information

A 'safeguarding serious case review' is not enough.

These have occurred before with other children who died in similar circumstances (Baby P being one of them) Lessons were obviously not learned.

We require an immediate judge-led public inquiry into the failure of so many agencies to protect this child.

Why did Police, Medical Professionals, Teachers and Social Workers not communicate their concerns within a multi-disciplinary team to build a complete picture of circumstances?

Why did professionals accept a diagnosis of a learning disability and eating disorder without evidence. Why didn't professionals ask for proof of the medication the child was supposed to be taking for the eating disorder?

Why did teachers not highlight a child losing weight and yet not bringing in adequate food to eat, witness him stealing food and witness him eating from bins?

Why were so many concerns ignored and not acted upon?
