Standardise ALL paperwork across the NHS

Additional Information

Standardising all paperwork across the NHS would prevent numerous errors and make it easier when transferring patients to different wards/hospitals and when staff move wards/hospitals. Often things are missed because staff do not know where to look for information or it is not made clear. Something so simple could make a big difference to out patients!
