Visitors visa,clarity with fees, routes of complaint & right of appeals needed.
All visitor visa fees be pre-warned.
With rejection rate as high as 50% in some Non-White countries.
Home office complaints only handling UK made applications, British High com, only nationals queries abroad, no admin review nor appeal rights
More safeguards needed to hold ECO's accountable.
Introduce a complaints route to dissatisfied foreign visa applicants.
UKVI be held liable for actions of its subcontractors.
Allow all visitors either ,
1) an admin review,
2) or a right of appeal.
Without a formal Complaints route, no Admin review / nor appeal rights to the FTT.
Current system lacks checks& accountability for decisions making resulting in abuse of power and impunity.
As a seller of service, UKVI should guarantee these as minimum standard of fairness at all costs.

We understand that you are concerned about visas, but we're not sure what you mean when you say that visitor visa fees need to be pre-warned. We're also not sure what safeguards you would like to put in place to hold ECOs accountable, and UKVI already has a complaints procedure in place.
