Refuse free healthcare to those who abuse drugs.
Those who smoke, drink to excess, or otherwise use drugs to an extent that is dangerous should not receive free healthcare as they are aware of the risks. They clog up the system whilst people with genuinely unavoidable conditions have to wait.
