I hereby petition Her Majesty's Government to legalise the practice of Assisted Suicide in the United Kingdom in situations in which:

1) the patient is physically and mentally capable of making the decision to end his/her own life, and has expressed a wish to do so; and

2) the patient has a terminal, chronic or disabling physical or mental illness which can be shown to cause him/her suffering should his/her life be forced to continue.
