Introduce independent accountability for SEND system failures
Children across England are being failed by the SEND system, including EHCP provision not delivered and children left without suitable education. There is no independent mechanism to investigate failures or ensure accountability when SEND duties are not met.
Families across England are living through serious failures in the SEND system, including EHCP provision not being delivered and children left without suitable education or support. When these failures occur there is no single independent mechanism to investigate systemic failures or ensure accountability. Children and families should not be left to carry the consequences when the systems designed to support them fail.

The Government e-Petitions Team gave the following reason:
We are not clear how this independent mechanism would be different to the First-tier Tribunal (Special Educational Needs and Disability) already in place.
You can start a new petition setting out more clearly how the action you are calling for differs from what is currently in place.
