Fund better rural post-diagnosis pathways for dementia patients & their carers
Fund improved post-diagnosis pathways for dementia patients and their carers in rural areas via GP practices, including specialised dementia care nurses, like Admiral Nurses, who can provide essential support, and more appointments for patients at their local GP within 7-10 days of diagnosis.
Memory Clinics, CST services, and other NHS resources are mostly in city hospitals. We believe resource should be shifted to rural areas through GP practices, which should have trained dementia care nurses (e.g. Admiral Nurses) for support. Patients should be offered a GP appointment within 7-10 days, including both the person with dementia and their carer or partner. A dementia nurse at the GP practice could also improve annual reviews. Smaller hospitals across the country could also host CST sessions.
